Job Title: Occupational Therapist/Physical Therapist

Location: G Road, Grand Junction, CO 81505
Duration: Permanent
Shift: Full-Time

Position Overview:
We are seeking a compassionate and dedicated Occupational Therapist (OT) / Physical Therapist (PT) to join our dynamic clinic team in Grand Junction, CO . This is a full-time position offering the opportunity to make a significant impact on patients' lives by providing high-quality, patient-centered care. As an OT/PT, you will assess, plan, and implement individualized treatment programs for patients of all ages, ensuring their needs are met through comprehensive care services.

Key Responsibilities:

• Patient Care Coordination:
• Evaluate and assess patients' needs, establish and implement a personalized plan of care, and modify treatment as needed to ensure successful outcomes.
• Work with patients and caregivers to educate on therapy goals, interventions, and progress.
• Treatment Planning & Modification:
• Assess treatment effectiveness and modify plans as necessary to achieve patient goals.
• Ensure comprehensive treatment is provided based on current professional and regulatory guidelines.
• Documentation & Data Analysis:
• Produce timely, high-quality documentation to support patient care.
• Collect and analyze data from outcome measures to track patient progress and improve therapeutic interventions.
• Collaboration & Communication:
• Maintain clear and open communication with physicians, staff, and hospital leadership to ensure coordinated care and high-quality service.
• Cost Efficiency & Budget Management:
• Demonstrate a solid understanding of treatment costs, including budgeting, staffing, and billing, and strive to optimize efficiency and quality in care delivery.

Requirements:

• Bachelor’s Degree or higher from an accredited Occupational Therapy program OR Physical Therapy program.
• Current BCLS (Basic Life Support) certification.
• Current, unrestricted license as an Occupational Therapist or Physical Therapist in the state of Colorado .
• Current Hand Therapy Certification from the Hand Therapy Certification Commission (HTCC) and recertification every five years.

Compensation:

• Hourly Pay Range: $48.00 - $55.20 per hour, based on education and experience.
• Additional Compensation:
• Discretionary bonuses
