Purpose of Position The purpose of this position is to provide free information and assistance to adults with memory or cognitive concerns, or who have a dementia diagnosis. Dementia Care Specialists also provide information and support to family members and friends who are providing care to a person with memory concerns. This position is responsible for creating a dementia capable Aging and Disability Resource Center (ADRC), dementia friendly communities, and provide dementia specific information and resources to providers, agencies, persons with dementia, caregivers, and the community.
The Caregiver Support Coordinator provides support to caregivers. Being responsible for someone else can feel overwhelming if the role is unexpected or interferes with job responsibilities. This is why Wisconsin offers family members and other informal caregiver programs and education opportunities that can make being a caregiver more manageable. The ADRC Caregiver Support Coordinator provides free and unbiased information and assistance to connect families with in-home care providers, respite services and other caregiver resources in the area. The coordinator is trained to navigate individuals through immediate crises and provide decision-making counseling so that caregivers can make informed decisions. The coordinator can also refer families to other professionals that provide legal advice and financial planning services, including information about Social Security, Medicare and Medicaid benefits. The coordinator can provide virtual and in-person services and support that help family members and informal caregivers, such as a neighbor or friend, care for older adults at home. The program prioritizes services to low-income families and older adults with dementia, but one does not have to be low income or have dementia to participate. It is available to any person providing care to an adult age 60+, or any grandparents or other relatives age 55+ who are the primary caregivers for youth under the age of 19.
Essential Functions To perform this job successfully, an individual must be able to perform each essential function satisfactorily and in a timely manner. The following duties are normal for this position. These are not to be construed as exclusive or all inclusive. Other duties may be required or assigned. Develop referral relationships with physicians, dementia diagnostic clinics and other health care providers.
Provide dementia-specific consultation and technical assistance related to cognitive screening and individual and family caregiver issues to adult protective services agencies (APS), crisis response systems, county/tribal aging offices, ADRC staff members, and other agencies, as requested.
Provide at least two approved evidence-based or evidence-informed interventions, one specifically designed for family caregivers, as part of ongoing activities. Maintain fidelity to the intervention model and accommodate families' schedules for meetings (e.g., evenings and weekends, when necessary).
Provide volunteer training and guidance to expand the reach of the goals of the Dementia Care Specialist (DCS) Program.
Collaborate actively and develop referral protocols with local, regional, and statewide dementia organizations including: the Wisconsin Alzheimer's Institute diagnostic clinics, the Alzheimer's Association chapters, and the Alzheimer's and Dementia Alliance of Wisconsin.
Offer and provide person-centered support when memory screens are positive to assure opportunities for follow-up with a primary care provider are available in accordance with the individual's preferences.
Offer and provide short-term service coordination for individuals with dementia and/or their caregivers. Coordinate with other ADRC, county/tribal aging unit(s), APS and other county staff members and be available for joint or independent home visits.
Serve as a catalyst for developing and implementing strategies to create and sustain dementia-friendly communities in the ADRC service area.
Provide outreach and awareness to professionals, employers, organizations, and the general community about the ADRC and available dementia services.
Maintain current knowledge of dementia, research findings, new evidence-based interventions, and other innovations for people living in the community.
Participate actively in DCS statewide and regional meetings, training programs, and conference calls. Assist colleagues by sharing experiences. Mentor new DCS staff and provide materials and products to other ADRCs, as requested and approved. Provide conference workshops as requested and appropriate to increase the awareness of dementia and services available.
Complete 100% time reporting, collect and report program data, and contribute to DCS program evaluation.
Maintain working knowledge of human service programs and applicable policies, manuals, guidelines, and statutes.
Attend and participate in unit, agency and County trainings and meetings.

Training, Experience and Other Requirements Bachelor's Degree in Social Work, Human Services, or related field, two years case management experience preferred, or any combination of education and experience that provides equivalent knowledge, skills, and abilities. Experience regarding older adults, dementia, care giver support, challenging behaviors, small group facilitation, public speaking, and development of collaborative relationships are preferred. Must be able to function independently and accommodate a flexible work schedule and environment to meet program needs. Valid motor vehicle operator's license and/or available means of transportation required. Social Workers are required to maintain their Wisconsin Social Work licensure or certification, to remain in the Social Worker position. Case Workers may obtain their Wisconsin Social Work licensure or certification and be eligible to advance to a Social Worker position.
