Sandra Eskenazi Mental Health Center, Indiana's first community mental health center, provides comprehensive care for emotional and behavioral problems, including severe mental illness and substance abuse. The Sandra Eskenazi Mental Health Center offers both inpatient and outpatient services, including several outreach centers as well as clinic- and community-based services.

Job Role Summary
The Liaison State Hospital ensures continuity of care for clients between Sandra Eskenazi Mental Health Center (SEMHC) and State psychiatric hospitals (SPH).
Essential Functions and Responsibilities
- Embraces, understands, and operates under the Recovery Model, including AIDET (Acknowledge, Introduce, Duration, Explanation, Thank You) and the spirit of motivational interviewing .
- Ensures continuity of care for clients between SEMHC and SPH
- Monitors and documents client progress
- Provides and documents direct service to SEMHC clients receiving treatment in SPH, including discharge planning with SPH and SEMHC
- Advocates for clients and ensures access to needed services, including developing and maintaining community relationships and resources
- Provides reports to Division of Mental Health and Addictions and other governmental agencies
- Provides assessment information for treatment planning and continuity of care
- Monitors and reports on court commitments for SEMHC
- Attends both SEMHC staffings and SPH staffings to assist in clients' linkage to community and provides timely communication with clients' families
- Tracks and provides reports to courts regarding commitment status of both SPH clients as well as SEMHC clients
- Monitors and ensures SEMHC stays under allocated SPH bed count
- Participates in SPH service committees as needed
Job Requirements
• Bachelor's degree in human services, psychology, social work, or a related field with two years of relevant work experience (i.e. social work, criminal justice, etc.) are required;
o Two (2) years of experience serving populations in a community mental health or psychiatric hospital setting preferred
o Experience working with both pediatric and adult populations preferred
- Master's degree in human services psychology, social work, or a related field preferred
- Valid Indiana driver's license and driving record that meets Eskenazi Health's driving policy requirements
- Valid Indiana behavioral health and human services (BHHS) license in good standing that meets Indiana BHHS licensure requirements is required, dependent upon the degree conferred (i.e. individuals with a bachelor's in social work must have valid Indiana Licensed Bachelor Social Worker license, etc.)
- Credentialed as a licensed clinical social worker in the State of Indiana preferred
Knowledge, Skills & Abilities
Knowledge
- Knowledge of the SEMHC crisis continuum, outpatient services, and acute care services
- Knowledge of SPH throughout Indiana
- Basic knowledge of substance use disorder and/or co-occurring disorders
- Knowledge of client rights and responsibilities
- Basic knowledge of psychotropic medication, their uses and side effects
- Must meet all population-specific competencies for role and service area
- Knowledge of trauma-formed, person-centered, family-centered, and recovery-oriented care.
